The New York State Department of Health over the past four years has barred 21 healthcare providers suspected of vaccine fraud from immunizing children, including nine who have practiced or lived on Long Island.
In some cases, investigators say, providers reported to the state's database that they used the same vaccines many times, or reported using vaccines with identifying numbers that didn't exist.
