Organic baby food company Once Upon a Farm is recalling a soft-baked bar product due to a potentially fatal choking hazard for young children.
The FDA warned customers to not buy or serve Once Upon A Farm Tractor Wheels Organic Toddler Soft-Baked Bars after receiving several reports of children ages 1½ to 2½ suffering airway obstructions after eating the product.
"When chewed, the product reportedly forms a dry, sticky mass that standard choking interventions — including back blows and the Heimlich maneuver — may fail to dislodge," stated an FDA warning on Friday.
Some reported incidents required emergency response and hospital intervention, the warning added.
According to the FDA, the agency informed the company of the reports of adverse events and Once Upon a Farm agreed to initiate a recall. The products should immediately be thrown away, the warning said.
The warning includes all "flavors, quantities, and varieties, including all lot codes and expiration dates," a notice on the FDA website said.
The soft-baked bars are sold nationwide in stores, including Costco, Target and Walmart, and online, including Amazon.com and other third-party vendors.
